Job summary

Eliot Community Human Services is seeking a Housing First Coordinator to join our Community Support Team, providing housing stabilization support to unsheltered adults transitioning to housing. The Housing First Model prioritizes permanent housing as the initial step, with person-centered coaching, education, and access to community resources to sustain housing and emotional well-being.

The role involves on-site office work and field visits with a client-centered approach.

Qualifications

• Bachelor’s degree in human services or social work or related field.
• Minimum 2 years case management with homeless population or lived experience with substance use disorder, homelessness, or mental illness.
• Massachusetts driver’s license and access to personally owned vehicle.

Responsibilities

• Provide person-centered, strengths-based case management.
• Coordinate housing stabilization services and safety planning.
• Advocate to ensure client access to support services.
• Engage clients onsite and in the community to support housing goals.
