We are seeking a Housing Coordinator for our Program for Assertive Community Treatment (PACT) program!
As a member of the PACT team, the Housing Coordinator will work as a member of Persons’ Team having primary responsibility for supporting person's served in attaining and keeping stable, secure housing. They will be responsible for educating staff and Persons using PACT services about tenancy issues, housing rights, and resources.
The Housing Coordinator will assist people served with applying for and attaining subsidies, and work with Persons to secure housing that meets their needs and will guide the Person in learning the skills necessary to maintain community housing. They will provide a single point of contact for developing relationships with landlords, housing authorities, and DMH Area housing staff, to foster communication and provide advocacy.
