Housing Community Supports Specialist / Job Req 822380162 (Manufacturing)

Remote: Full Time, must live in one of the 6 states where we operate: CA, AZ, NV, TX, WA, OR. Available for Full-Time Work Schedule 8:00am - 5pm Pacific Time, Monday-Friday.

Principal Responsibilities:

The Alameda Alliance for Health (Alliance) Housing Community Supports Specialist is dedicated to bridging homelessness, housing, and healthcare. Under the supervision of the Housing Program Manager, the Specialist collaborates with internal teams such as Long Term Services and Supports, Clinical, Behavioral Health, ECM, Healthcare Services, and Case Management, as well as external partners including Alameda County Continuum of Care, Alameda County Health, and community-based organizations (CBOs). The role involves providing social service linkages, reviewing housing needs assessments, and supporting members' transition into housing, emphasizing diversity, racial equity, inclusion, and belonging.

Key responsibilities include:

  • Processing approvals/denials for Housing CS requests via the enterprise care management platform.
  • Notifying members and providers about authorization outcomes.
  • Facilitating communication between providers, vendors, CBOs, ECM, and Alliance Case Management.
  • Using established guidance pathways for screening and approval processes.
  • Data entry and tracking of cases.
  • Interpreting and communicating Housing CS services and verifying member eligibility.
  • Prioritizing approvals and coordinating with the Housing Program Manager.
  • Identifying community-based resources to support independence and health outcomes.
  • Working with local agencies and landlords to identify housing options.
  • Coordinating referrals and resource linkages with community providers.
  • Supporting client-centered, integrated service models.
  • Reviewing housing needs assessments and support plans.
  • Managing caseloads, reviewing progress, and closing cases as appropriate.
  • Conducting outreach, training, and community engagement to promote programs.
  • Participating in team meetings and training sessions.
  • Ensuring compliance with policies, procedures, and legal requirements such as HIPAA.
Physical Requirements:
  • Constant visual work, sitting, and data entry.
  • Frequent use of telephone and verbal communication.
  • Frequent lifting up to 30 lbs., walking, and standing.
  • Travel within Alameda County as needed.
Minimum Qualifications:
  • Bachelor’s degree in related fields or equivalent experience.
  • Preferred licensure: LCSW, LMFT, LPCC.
  • Two years experience in housing support services and working with co-morbid populations.
  • Experience with healthcare/housing software systems.
Skills and Abilities:
  • Deep knowledge of Alameda County community resources.
  • Experience with Medi-Cal, Coordinated Entry, and homeless services standards.
  • Skills in barrier removal, Harm Reduction, Housing First, Trauma-Informed Care, Crisis Intervention.
  • Strong interpersonal, communication, organizational, and computer skills.

Salary Range: $83,241.60 - $124,862.40 annually.
