Job summary

Helpline-Youth-Counseling seeks a Housing Case Manager to develop individualized service plans and coordinate housing-focused case management under CalAIM. You will assist adults and families, advocate for benefits, and connect clients with housing options, mental health services, and support programs.

Responsibilities include documenting services, coordinating with landlords, attending meetings, and educating clients on leases and budgeting.

Qualifications

  • Bachelor’s degree in social services preferred; high school diploma required.
  • One year of experience with homelessness, mental health, or substance use challenges.
  • U.S. residency or citizenship required; ability to pass background and TB screening.

Responsibilities

  • Develop and implement individualized service plans addressing barriers and goals.
  • Advocate to secure benefits and resources for client self-sufficiency.
  • Provide housing placements, referrals, and connections to mental health and care management.
  • Document services accurately and timely as contractually required.
  • Assist with vital documents (birth certificates, SSN, ID).
  • Help locate housing options and liaise with landlords and property managers.
  • Attend internal/external meetings and trainings; support workflow improvements.
  • Maintain client contact via in-person, email, phone, and outreach attempts.
  • Educate clients on leases, budgeting, and essential life skills.

Job description

The Housing Case Manager role is a case management role that works directly with program participants to develop, arrange, and coordinate housing focused case management services through the CalAIM (California Advancing and Innovating Medi-Cal) Community Supports Program. Participants are adults and children with families. The primary focus is to provide comprehensive Housing Navigation and Housing Tenancy case management services to members who have chronic health and/or mental health conditions who may be experiencing homelessness or at risk of homelessness. The Housing Case Manager would be responsible for all case management activities associated with the designated program.DUTIES/RESPONSIBILITIESDevelop and implement individualized service plans tailored to each client's needs, including addressing barriers, setting short- and long-term goals, and action steps to achieve housing stabilityAct as an advocate for clients to ensure maximum benefits and resources are obtained and maintained to ensure client’s ability to become self sufficientProvide case management services, including but not limited to housing placements, referrals to resources needed, connection to mental health services, connections to enhanced care management services, substance use referrals, and other services as neededWork with the manager and director to provide constructive feedback on workflows, outreach techniques, and processes.Maintain accurate and timely documentation of all services provided and as required by contractsAssist clients in obtaining vital documentation such as birth certificates, social security cards, and identificationAssist clients with locating viable housing options, linkage to interim/emergency housing, and filling out relatable documentationDevelop relationships with property managers and landlords as well as local organizations to streamline referrals when neededAttend all internal and external meetings and trainings as directed by leadershipMaintain contact with clients utilizing different modalities of contact including but not limited to in person visits, emails, phone calls, and attempt multiple outreach attempts defined by the contract if unable to contact the clientProvide ongoing case management services once a client is housed and assist with maintaining housingEducate clients on essential skills like understanding lease agreements, communicating effectively with landlords, budgeting for rent and utilities and other life skills as neededMaintain accurate and timely documentation of all job-related activities.Coordinate transportation and accompany members to critical appointments as needed.Performs other duties as assigned by managerREQUIREMENTS:Bachelor’s degree in social services field preferred. High school diploma requiredOne year’s experience working with individuals experiencing homelessness, mental health issues, substance use disorders, and physical disabilitiesPermanent U.S. residency or citizenshipAbility to pass criminal clearance background checkAbility to successfully pass a TB screening testValid California state driver’s license, current auto insurance, and a good driving recordExcellent time management skills, ability to manage multiple tasks, projects and prioritiesAbility to work a flexible schedule, with evenings and occasional weekends as needed. The typical schedule would be Monday-Friday 8am-4:30pmBilingual in English and Spanish highly desired but not requiredTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
