Job summary

The Road Home is seeking a compassionate Housing Case Manager to join ourSalt Lake County team. You will partner with families who have moved out of homelessness to secure stable housing and access supportive services.

You will work in homes and in the field, coordinating with agencies, schools, and landlords to meet diverse needs. This role emphasizes trauma-informed care and tenant-focused empowerment.

Qualifications

  • Bachelor’s Degree or equivalent experience preferred.
  • Must pass pre-employment background check and drug screening.
  • Physical ability to lift up to 50 pounds and drive as needed.

Responsibilities

  • Provide direct case management to families, including crisis management, ongoing assessment, goal setting, and problem solving.
  • Connect families with housing resources, rental assistance, and community referrals.
  • Meet families in their homes and community locations to assess needs and support.
  • Maintain accurate case notes and participate in team meetings and trainings.

JOB SUMMARY

The Housing Case Manager will work as part of our Housing Case Management Team to provide direct Case Management services to families who have moved out of homelessness and are living in housing throughout Salt Lake County. The Case Manager will help connect families with resources and work closely with the Case Management Team, other agencies, and community partners to help support andfacilitatefamilies in meeting their diverse needs. Case Management duties will include time in the office, in families’ homes, and in agency vehicles transporting clients throughout the county. Services will be provided with a strong emphasis on Trauma Informed Care and Housing Focused principles.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Work with families receivinglong-term rental assistance to help thembecome stabilized in their housing and community.
  • Focus on housing and strength-based practices to meet the unique needs of each tenant.
  • Meet with families in their home to better assess family dynamics, needs, and support.
  • Provide services to a caseload of around 25 families living throughout Salt Lake County.
  • Familiarity with rentalassistancefunding sources.
  • Provide clients with program referrals and links to other communityresources, andfacilitate each client’s access to these resources.
  • Provide direct case management to families,including:crisis management, ongoing assessment, goal setting, and problem solving.
  • Assistclients in obtaining employmentand orworking on applying for Social Security benefits.
  • Advocate and negotiate on the client’s behalf with property managers.
  • Assess children’s needs to makeappropriate communityreferrals, andliaise with schools as needed for supportive services.
  • Assisttenants in getting on waitlists for Housing Choice Vouchers, Public, and other Subsidized Housing options
  • Attend weekly staff meetings, case staffing, andadditionalmeetings asassigned.
  • Engage in daily collaboration with other departments,maintainingand promoting a healthy team environment.
  • Keepaccuraterecords, case management notes, and statistical data as assigned.
  • Attend mandatory agencytrainings, including Trauma-Informed Care,HousingFocused, De-escalation, Suicide Prevention, CPR, and more.
  • Participate in emergency drillsand environmental safety activities,asrequired.
  • Follow all agency protocols for security, safety, and sanitation tomaintaina clean and secure environment for guests and staff.
  • Maintain regular and reliable attendance as an essential function of this position.

EDUCATION AND EXPERIENCE

PHYSICAL AND EQUIPMENT REQUIREMENTS
  • Ability to lift and move heavy itemsup to 50 pounds, including food boxes andclientsbelongings
  • Ability to sit, stand, andmovefor at least an hour at a time or more.
  • Ability to bend, squat, kneel, twist, push, and pull to perform job functions.
  • Ability to use stairs or steps.
  • Must be at least 21 years old, have avalid, unexpired driver’s license,and the ability to drive a vehicle, as needed.
