Job summary

Centerstone is seeking a Housing Case Manager in Hannibal, MO, to support individuals and families facing homelessness through Housing First and low-barrier services. You will collaborate with landlords, community partners, and service providers to connect clients with housing opportunities and ongoing supports.

Responsibilities include coordinating entry processes, documenting outcomes, and helping clients access benefits, healthcare, and employment services, while maintaining confidentiality
