Job summary

Northwest Michigan Community Action Agency, Inc. seeks a Housing Based Case Manager for Traverse City to support individuals and families experiencing homelessness toward safe, permanent housing.

This hands-on role involves community outreach, field visits, and coordinated services across Antrim, Benzie, Grand Traverse, Kalkaska, and Leelanau counties. You will manage up to 30 households, develop personalized housing plans, connect clients with resources, and document progress in HMIS.

Qualifications

  • Bachelor's degree in social work, psychology, human services, or a related field preferred.
  • Two years of relevant experience preferred.
  • Experience with homelessness, housing instability, or other complex social service needs is a plus.

Housing-Based Case Manager - Housing Stability Programs

Northwest Michigan Community Action Agency (NMCAA) is looking for a Case Manager to join our Housing Stability Programs team. This is a hands-on, community-based role supporting individuals and families experiencing homelessness as they work toward safe, permanent housing.

What You'll Do

  • Build trusting, person-centered relationships with individuals and families experiencing homelessness.
  • Meet clients where they are, including homes, shelters, community locations, and outdoor settings.
  • Participate in community and street outreach to connect individuals experiencing homelessness with available resources and services.
  • Manage a caseload of up to 30 households, depending on program needs and level of client support.
  • Develop housing and stability plans based on each client's strengths, goals, and needs.
  • Help clients identify housing options, overcome barriers, and work toward obtaining and maintaining stable housing.
  • Connect clients with community resources, such as financial assistance, mental health services, employment supports, and other programs.
  • Work with landlords, shelters, service providers, and community partners to resolve challenges and support housing stability.
  • Participate in case conferencing and coordinated community efforts to address homelessness.
  • Maintain accurate and timely client records, case notes, housing plans, and assessments in the Homeless Management Information System (HMIS).
  • Complete required program and grant documentation.
  • Participate in required training and professional development.

Qualifications

  • Bachelor's degree in social work, psychology, human services, or a related field preferred.

  • HMIS experience preferred.
  • Strong computer, communication, organization, and documentation skills.
  • Valid driver's license, vehicle insurance, and reliable transportation required.
  • Ability to travel throughout the service area and work in community settings year-round.

Full-time | Monday-Friday, 7:30 a.m.-4:30 p.m.

Work setting: Combination of remote, in-office, and community-based work depending on program and client needs.

This position provides coverage throughout Antrim, Benzie, Grand Traverse, Kalkaska, and Leelanau counties, with regular travel required to meet with clients and community partners.
