Required: Qualified Mental Health Associate (QMHA)
Job Summary
The Housing and Benefits Specialist will work with adults diagnosed with severe and persistent mental illness (SPMI) as part of a multidisciplinary team dedicated to providing whole-person care. The role involves helping participants access, apply for, and maintain public benefits, and supporting those who are unhoused or precariously housed in finding and securing appropriate housing. The specialist will also lead a Rent Well Group and serve as the key contact for ongoing voucher support.
Key Responsibilities
- Assist participants with applications for benefits like SSDI, SSI, Medicaid, Medicare, SNAP, and other entitlements.
- Transport and accompany individuals to agencies.
- Help obtain identification and necessary documents.
- Coordinate appointments and support participants during meetings.
- Act as liaison with agencies managing benefits.
- Follow up to ensure benefits are maintained, including recertifications and appeals.
- Explain benefits options and eligibility to participants.
- Support participants in finding and applying for housing programs.
- Manage housing vouchers and coordinate with landlords and agencies.
- Conduct housing assessments and develop support plans.
- Document interactions and progress.
- Stay informed on policy changes affecting benefits and housing.
- Advocate for participants with complex systems and issues.
- Assist with other needs related to financial stability and healthcare.
- Host Rent Well Group sessions.
- Manage housing data in HMIS, HSRN, and PowerBI.
- Support clinic functions and team meetings.
Qualifications
- QMHA qualification or ability to obtain via Bachelor’s in social work, human services, or 3 years relevant experience.
- Strong organizational and communication skills.
- Ability to work effectively with a multidisciplinary team.
- Valid driver’s license.
Preferred Qualifications
- Experience in social services, benefits coordination, or housing services.
- Knowledge of public benefits and housing programs.
- Experience with individuals with mental illness and trauma-informed approaches.
- Experience leading support groups.
