This role offers a flexible schedule with no on-call, weekends, or holidays required, and involves approximately 85% travel across nine southeastern states. All travel expenses are covered, and the compensation includes a competitive base salary plus incentives.

Responsibilities

• Travel up to 100% across 9 states, conducting in-home assessments including medical history, physical exams, and screenings.
• Identify diagnoses and coordinate follow-up care, communicating findings to patients' PCPs.
• Identify urgent situations and educate patients on health topics.

Qualifications

• Active ANCC or AANP certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care/Acute Care.
• At least 1 year of clinical practice as an NP.
• Willingness to travel extensively within the region.
• Proficiency with EMRs and internet; valid driver’s license and reliable transportation.

Preferred Skills

• Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
• Home health or home visit experience.

Compensation and Benefits

Salary ranges from $89,800 to $176,700 annually, based on experience and location. Benefits include comprehensive packages, incentives, stock purchase options, and 401(k). All applicants must meet employment eligibility criteria, and the role requires a commitment of at least 12 months in the position.
