As a certified Great Place to Work, CAIPA Care is excited to share an opportunity for Physician Assistants and Nurse Practitioners to join our fast-growing team as a Clinical Advisor. We are looking for a Physician Assistant or Nurse Practitioner to join our house calls team to provide support and training to our team of Advanced Practice Providers (APP) in addition to conducting health risk assessments on our patient population. These assessments take a holistic approach to help close gaps of care and address medical, social, and psychological needs to improve health outcomes in the comfort of the patient’s own home. This position requires in-home patient visits and will entail travel and autonomy with flexible scheduling.
Job Responsibility
- Supervise a team of APPs to maintain program quality including clinical metrics and patient satisfaction
- Train new hire APPs in health risk assessments and appropriate triage skills
- Serve as clinical expert to fellow APPs, medical team members, and staff
- Perform in-home health assessments of high-risk patients including medication review, Point-of-Care testing, review of systems, and physical examination leading to a comprehensive picture of a patient’s medical conditions
- Identify opportunities for preventive care, health promotion, home safety, and illness prevention
- Educate and emphasize the importance of consistent monitoring of health needs together with their Primary Care Physician (PCP) and provide patient education regarding health conditions, medications, and nutrition
- Communicate urgent medical needs and collaborate with the patient’s PCP for clinical escalation and care management needs as needed
- Thinking critically to address patient needs and provide the appropriate resources
- Comply with all HIPAA regulations and maintain active licenses and certifications
- Flexible weekly scheduling with no call, nights, or weekends
Requirements :
Qualification and Education
- Physician Assistant with a BS / MA Degree from an ARC-PA accredited Physician Assistant Program OR a Master level or above degree from an accredited Nurse Practitioner program
- BLS certification
- New graduates are welcome, 1+ year(s) experience in Family Practice / Internal Medicine / Geriatrics preferred.
- Bilingual in English and Chinese (Mandarin or Cantonese) required
