Salary Range: $18,000.00 to $65,000.00/year depending on the client’s disability level

Host Home Provider Position

We seek experienced Host Home Providers willing to open their homes to individuals who can no longer live independently and prefer not to reside in communal settings. Providers share their homes and lives, offering a private bedroom, hands-on support, and ongoing daily care. You can help individuals with medical concerns or behavioral challenges and make a meaningful difference in their lives by becoming part of your family. Provider Duties and Responsibilities:

1. Provide personal care, including hygiene assistance, meal preparation, and dressing.
2. Assist with employment coaching to help clients find jobs.
3. Offer recreational activities to promote social development.
4. Include clients in family activities and daily life.
5. Accompany clients to outings such as supermarkets, museums, parks, dances, and recreation centers.
6. Provide transportation as needed.
7. Assist with medical care, including appointments and medication administration.
8. Maintain documentation of progress notes and medical records.
9. Provide training on life skills and learning activities.

Qualifications for Success:

• High school diploma or equivalent (Required)
• Complete and pass criminal and motor vehicle background checks
• Complete training for Medication Administration, CPR/First Aid, and maintain ongoing training requirements
• At least 1 year of experience as a host home provider
• Experience with behavioral needs (preferred)
• Experience with medical needs (preferred, e.g., CNAs, LPNs, RNs)
