HOST HOME PROVIDER POSITION

We are seeking experienced Host Home Providers willing to open their homes and foster individuals who can no longer live independently in their community and prefer not to reside in a communal setting. Providers share their home and lives with individuals with developmental disabilities. As a Host Home Provider, you will take the client into your home, providing a private bedroom, hands-on support, and ongoing daily care. You can help someone with significant medical concerns or behavioral challenges and make a meaningful difference in their life as they become part of your family. HHP Provider Duties and Responsibilities:

• Provide personal care including assistance with hygiene, meal preparation, dressing
• Assist with employment coaching to help clients find jobs
• Organize recreational activities to promote social development
• Include clients in family life and activities
• Accompany clients to supermarkets, museums, parks, dances, recreation centers, etc.
• Provide transportation as needed
• Assist with medical care, including appointments and medication administration
• Document progress notes and medical records
• Provide training on life learning skills

To be successful, candidates should:

• Have a high school diploma or equivalent (Required)
• Complete and pass criminal and motor vehicle background checks
• Complete training for Medication Administration, CPR/First Aid, and maintain ongoing training requirements
• Have at least 1 year of experience as a host home provider
• Preferably experienced with behavioral needs
• Preferably experienced with medical needs (e.g., CNAs, LPNs, RNs)
