Overview

A Host Home provider is an independent contractor who opens their home and heart to an adult with a developmental disability. Supports may include assistance with personal care, hygiene, meal planning, cooking, medications, recreation, leisure activities, transportation, shopping, and more.

Responsibilities
  1. Provide daily care, activities, and supervision for the Individual according to their Service Plan and legal requirements.
  2. Prevent acts of disrespect, abuse, neglect, or exploitation; report any incidents immediately.
  3. Document progress and events; complete reports as required by law.
  4. Arrange medical and dental care respecting the Individual’s needs.
  5. Transport the Individual to appointments and community activities.
  6. Notify supervisors immediately in case of serious health issues or death.
  7. Maintain confidential records and communicate regularly with the company.
  8. Complete daily notes and attend scheduled meetings.
  9. Assist with training relief staff and ensure compliance with training documentation.
  10. Report changes in the Individual’s condition or environment.
  11. Review and sign medication administration records (MARs) and related documentation.
  12. Coordinate necessary medical orders and follow-up on labs and tests.
  13. Ensure all medical and safety equipment is up-to-date and functioning.
  14. Participate in annual health checks and follow-up appointments.
  15. Perform limited travel as needed and other duties as assigned.
Qualifications
  • High School Diploma or GED
  • Training in CAP Core Competencies
  • Valid driver’s license, reliable vehicle, good driving record
  • Effective verbal and written communication skills
  • Comfort with assisting residents with hygiene and medical routines
  • Strong organizational and interpersonal skills
