Job summary

Continuum of Colorado is seeking compassionate Host Home Providers in Boulder, Thornton, Broomfield, Longmont, or nearby areas to support adults with intellectual and developmental disabilities in a safe and caring home environment.

As a contracted provider, you’ll open your home and your heart to help someone thrive in a community-based setting, with duties including daily living support, transportation, and documentation. Contracted providers are not eligible for company benefits.

Qualifications

• Must be 18 years of age or older.
• Pass a criminal background check.
• Complete all state-required trainings and certifications.
• Maintain liability insurance as outlined in contract.
• Comply with state and federal regulations for residential services.

Responsibilities

• Assist with daily living tasks such as personal care, hygiene, meal prep and medication management.
• Provide behavioral support and help with medical or safety needs.
• Encourage community involvement, recreation and life skills development.
• Offer transportation to appointments and activities.
• Document supports and participate in monitoring or team meetings.
• Ensure a supportive, person-centered environment focused on independence and quality of life.

What You'll Do

As a Host Home Provider, you’ll offer individualized support to a person with a developmental disability living in your home. This includes:

• Assisting with daily living tasks such as personal care, hygiene, meal prep, and medication management
• Providing behavioral support and helping with medical or safety needs
• Encouraging community involvement, recreation, and life skills development
• Offering transportation to appointments and activities
• Documenting supports and participating in monitoring or team meetings
• Ensuring a supportive, person-centered environment focused on independence and quality of life

Requirements

Monthly pay rate: $3,000 - $6,000/month depending on level of care of individual(s)

Contractor Requirements

• Must be 18 years of age or older
• Pass a criminal background check
• Complete all state-required trainings and certifications
• Maintain liability insurance as outlined in contract
• Comply with all state and federal regulations for residential services

Continuum of Colorado is particularly seeking Host Home Providers who can offer wheelchair-accessible homes, and those with the capacity to support individuals requiring intensive behavioral or medical care.

The contract rate for this position is reflective of the level of support required by the individual you will be working with. Individuals with higher support needs will have a corresponding higher rate, while those with lower support needs will have a more moderate rate. This structure ensures that compensation is aligned with the unique requirements of each individual’s care.

Contracted Host Home Providers are contractors and not employees; therefore, they are not eligible for company benefits.
