The Clare in Chicago is seeking a hospitality-focused Licensed Practical Nurse (LPN) to deliver direct nursing care and supervise care provided by our Quality-of-Life Specialists/CNAs. This full-time role offers day or night shifts with a rotating weekend schedule.

Responsibilities include assessing resident needs, developing and monitoring care plans, reporting to Health Services leadership, and coordinating with other disciplines to ensure high-quality, compassionate care.

Qualifikationen

• Current nursing license valid in Illinois.
• Willingness to be available for any/all emergencies regarding the community.

Aufgaben

• Assist residents with Activities of Daily Living (ADL).
• Identify resident medical/behavioral changes and develop/reassess care plans.
• Provide daily report to Health Services on conditions and needs.
• Deliver nursing care per physician orders and established standards.
• Coordinate with other disciplines to ensure services are provided.
