The Mount Sinai Health System seeks a physician to cover post-acute and long-term care at The New Jewish Home Skilled Nursing Facility in New York City. This role sits within the Division of Hospital Medicine and is based at The New Jewish Home on the Upper West Side in Manhattan.

Training in Internal Medicine or Family Medicine is required; geriatrics/palliative certification is preferred. Compensation ranges from 198K to 210K for full-time employment.

Formación

• Must have Medical Degree from an Accredited University.
• New York State Medical License required.
• Board Eligible or Board Certified in Internal Medicine or Family Medicine.
• Geriatrics and/or Palliative Medicine certification preferred.

Responsabilidades

• Completes comprehensive admission evaluations of all new patients.
• Effectively manages chronic medical conditions to maximize recovery.
• Identifies and addresses new medical conditions promptly.
• Collaborates with patients, families and interdisciplinary teams to meet goals.
• Ensures seamless care transitions and discharge planning.
