Qualifications

Responsibilities

• This role provides patient-focused pharmaceutical care by reviewing and monitoring medication therapy to ensure safe, effective, and cost-conscious treatment.
• Responsibilities include medication order verification, dosing adjustments, medication reconciliation, patient education, and participation in multidisciplinary patient care rounds.
• The pharmacist will also support medication safety and performance improvement initiatives, provide education to staff, and assist with departmental coverage as needed.
