Endeavor Health is hiring an Advanced Practice Provider (APN/PA) for the Hospital Medicine Department at Swedish Hospital in Chicago. The role is full‑time with 12pm–12am shifts, and occasional 7pm–7am coverage.

Involves inpatient care, autonomous decision making within protocols, and collaboration with physicians to deliver high‑quality patient care. Illinois APN/PA licensure and DEA/IL CS licenses are required, along with a master's degree and BLS/ACLS certification.

Qualifications

• IL APN or PA licensure required; DEA and IL Controlled Substance license required.
• Masters level degree in specialty area.
• BLS/CPR and ACLS certification required.

Responsibilities

• Provide care to inpatients under physician supervision.
• Obtain detailed history and perform physical exams.
• Order imaging, labs, and diagnostic procedures and interpret results.
• Instruct patients on therapies and pre/post operative care.
• Maintain records and write progress notes.
• Collaborate with physicians and health-care team to optimize outcomes.
• Prescribe pharmacologic and non-pharmacologic therapies as appropriate.
• Participate in standards of practice development and evaluation.

• Exciting opportunity to join Endeavor Health in the Hospital Medicine Department.
• Under the direction of the collaborating physician(s), provides care to patients.
• Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results.
• Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions.
• Maintains records and writes progress notes on status, treatments and procedures.
• Follows patients through their course of treatment in the clinic and hospital setting.
• Collaborates with physicians and the health care team to achieve optimal patient outcomes.
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate.
• Demonstrates evidence-based practice.
• Participates in the development, implementation, and evaluation of standards of practice in the specialty.
• Assesses the learning needs of patients and families across the continuum.
• Employs strategies, interventions and teaching to promote, maintain, and improve health.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities.
• Works autonomously and in interdisciplinary teams as resources and consultants.
• Positively contributes to safety outcomes and promoting high quality patient experience.
• Participates in the development, implementation, and evaluation of service line goals.
• Supports the strategic plan of the organization.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of masters level program in specialty area
• Certification: BLS/CPR and ACLS certification required
• Experience: Prior Hospitalist APN/PA experience highly preferred
