Job summary

Yale New Haven Health System Hospitalist Service is seeking APRNs or PAs to provide comprehensive inpatient care under the supervision of attending physicians. The APPs will document in EPIC, round daily, and coordinate with rehab and other services.

Discharge planning, patient education, and follow-up coordination are integral to the role. Requires PA or NP certification and CT licenses, with prior acute care inpatient experience preferred.

Qualifications

• Graduate from an accredited Physician Assistant program.
• Certified as a Physician Assistant by the NCCPA or equivalent.
• Nurse Practitioner degree (BSN/MSN) and certification by ANCC or equivalent.
• CT medical license and DEA registration required.

Responsibilities

• Assist in care of medicine inpatients under supervising physicians.
• Document medical records, order tests and coordinate with consultants.
• Round daily and communicate patient status and plan with attending physician.
• Coordinate discharge planning and complete discharge paperwork.

The Hospitalist Service at Yale New Haven Hospital is actively seeking APRNs or PAs. The primary function of the Advance Practice Provider (APP) working for the Hospitalist Service is to provide comprehensive care to medicine inpatients in collaboration with attending physicians and under the direction of the APP Manager(s) and Director of the Hospitalist Service. Each APP has a designated supervising/collaborating physician.

Clinical Practice

• At the instruction of the APP Manager and Director or his/her designee, assists in the care of medicine inpatients assigned to the Hospitalist Service, ensuring safe, high quality, patient-centered and efficient care.
• Collaborates with attending physicians (Hospitalist Service or Private) to execute patient care. Holds primary responsibility for medical record documentation, ordering tests/procedures and communication with consultants.
• Identifies self in EPIC as covering provider by 8:30a, or earlier if patient assignments are available.
• Writes name daily on the patient's whiteboard to identify the practitioner taking care of them on any given date.
• Rounds daily on assigned patients and regularly communicates patient status, results and changes in plan of care to the attending physician.
• Ensures all patient handoff sections are updated in EPIC for assigned patients by 6:00 p.m.
• Services as a liaison and interacts with rehabilitation staff, food and nutrition staff, and other ancillary services to ensure timely and comprehensive care for all patients. Attends 9 a.m. huddles and transition of care rounds daily.
• Serves as a liaison to patients and their families.
• Educates patients and families about transition needs. Completes discharge paperwork, including the W-10, prescriptions and the discharge summary (pending staffing model). Reviews discharge instructions with patients and nursing at time of discharge. Coordinates outpatient follow-up.
• Performs necessary invasive diagnostic and therapeutic procedures, as credentialed by Yale-New Haven Hospital Office of Physician Services or under the supervision of an Attending physician.

Qualifications

Education

Physician Assistant: Graduate from an accredited Physician Associate program required. Certified as a Physician Associate by the National Commission on Certification of Physician Associates.

Nurse Practitioner: Bachelor of Science or Bachelor of Art degree and graduation from an approved Nurse Practitioner program required. Certified as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or another specialty nursing organization such as the Oncology Nursing Certification Corporation.

Both hold a valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification.

Experience

One (1) to two (2) years caring for medicine inpatients preferred.

Acute Care training required.
