Yale New Haven Health is seeking APRNs or PAs to join the Hospitalist Service to provide comprehensive inpatient care in collaboration with attending physicians. The APPs will document in EPIC, order tests, coordinate with consultants, and participate in rounding and care transitions.
The role emphasizes patient-centered care, collaboration with multidisciplinary teams, and discharge planning with outpatient follow-up. Acute Care training and CT licensure are required.

The Hospitalist Service at Yale New Haven Hospital is actively seeking APRNs or PAs. The primary function of the Advance Practice Provider (APP) working for the Hospitalist Service is to provide comprehensive care to medicine inpatients in collaboration with attending physicians and under the direction of the APP Manager(s) and Director of the Hospitalist Service. Each APP has a designated supervising/collaborating physician.

EDUCATION
Physician Assistant: Graduate from an accredited Physician Associate program required. Certified as a Physician Associate by the National Commission on Certification of Physician Associates.
Nurse Practitioner: Bachelor of Science or Bachelor of Art degree and graduation from an approved Nurse Practitioner program required. Certified as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or another specialty nursing organization such as the Oncology Nursing Certification Corporation.
Both hold a valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification.
EXPERIENCE:
One (1) to two (2) years caring for medicine inpatients preferred.
Acute Care training required.
