Job summary

Responsibilities

• Interview patients and families to assess home environment and health history.
• Establish and maintain effective working relationships with patients, families, staff, and community resources.
• Assess patient potential for home maintenance and needed services; coordinate appropriate services.
• Intervene as a crisis counselor when immediate psychological treatment is required.
• Refer patients to community resources and follow medical recommendations.
• Provide ongoing counseling and support to patients and families dealing with advanced illness, death, and bereavement.
• Write reports and correspondence.
