• Assist with discharge planning in collaboration with the interdisciplinary team to develop safe, realistic discharge plans.
• Collaborate with case managers and clinical teams to determine appropriate treatment plans and resources.
• Provide information and referrals for community resources and support advance directives as needed.
• Maintain and submit required documentation and participate in rounds and committee meetings.
