Job summary

Qualifications

• Master's degree in Social Work required; licensure may be required by location.
• OAPSA clearance may be required depending on location.
• LSW or LCSW eligibility preferred depending on facility requirements.
• Minimum 2 years' hospital or multidisciplinary healthcare experience.
• Background clearances: Act 34, Act 33, and Act 73 FBI fingerprinting.

Responsibilities

• Completes and documents psychosocial assessments, interventions, and care coordination.
• Provides crisis intervention and counseling to patients to improve coping.
• Engages patient/family and care teams in discharge planning and post-acute resources.
• Educates patients and teams about psychosocial factors related to illness.
• Participates in advancing the hospital's social work role and practices.
• Performs other duties as assigned.
