Job summary

UnityPoint Health in Des Moines, IA, is seeking a Hospital Social Worker to join our interdisciplinary care team. You will assess psychosocial needs, provide crisis intervention, educate on directives, assist with transitions, and connect patients with community resources.

The role requires a BSW (MSW preferred), licensure within 1 year, and travel as needed. Two openings available with a Monday–Friday schedule, 8:00 a.m.–4:30 p.m., on-site at UPH facilities.

Qualifications

• BSW degree from an accredited school of social work.
• MSW degree in Social Work preferred.
• One year clinical experience in health care preferred.
• Valid licensure (LBSW, LMSW, LISW or similar) within 1 year of hire.
• Complete Person Centered Care course within 1 year of hire.
• Mandatory Reporter course completion as required by state.

Responsibilities

• Performs psychosocial assessments to identify priority needs, strengths, and barriers to care.
• Provides crisis intervention and support to patients and families.
• Educates patients and families about Advanced Directives and facilitates care planning conversations.
• Assists with planning for care transitions and collaborates with community services for continuity of care.
• Maintains knowledge of community resources and acts as a liaison to refer to health and social services.
• Documents assessments, interventions, and referrals in the EHR according to standards.
• Mentors new employees and social work interns as requested.
