Job summary

Qualifications

Responsibilities

• Screens charts and conducts psychosocial evaluations through interviews with patients and surrogates.
• Provides individual and group counseling within health care settings or community settings as needed.
• Collaborates with professionals from other disciplines to develop multidisciplinary treatment plans.
• Prepares patients for services, makes referrals and assists with discharge and aftercare planning.
• Acts as a mandated reporter and documents cases of abuse as required by law.
• Maintains records and reports, contributes to QA/PI activities.
• May provide clinical services under supervision of a NYS-licensed clinician.
• Mentors less experienced staff and may supervise students or volunteers.
