Job summary

Optum, in collaboration with ProHealth Care, seeks a Social Worker in Waukesha, WI, to coordinate care for medically complex patients and connect families to resources. You will work with an interdisciplinary team to plan discharge, provide counseling, and advocate for patient wellbeing.

Flexible schedule and strong support structure offered. Minimum BSW with 2+ years of healthcare experience is required; preferred MSW.

Qualifications

• Bachelor's degree in social work (BSW).
• Social Work Certification.
• 2+ years of experience working in a healthcare setting.
• 2+ years of experience/knowledge in healthcare terminology.
• Ability to work onsite at hospital location(s) in Waukesha WI regional area.

Responsibilities

• Coordinates patient care processes to achieve desired quality outcomes and identifies / controls appropriate resource utilization.
• Provides skilled interventions for patient/family support and crisis resolution; facilitates problem solving and advocacy.
• Assesses need for post-hospital services and community resources at discharge.
• Develops care plans and management plans with physicians and ED/providers.
• Supports patients, caregivers and families with psychosocial or behavioral needs.
• Educates patients and families on options within the appropriate level of care.
