Qualifications

• Master's degree in Social Work with Healthcare-based practicum.
• Licensed in Washington as LSWAIC with commitment to LICSW within four years.
• Hospital or equivalent experience preferred; experience prior to obtaining a Master's degree may be considered.
• Health industry knowledge preferred.
• Healthcare financial assistance knowledge preferred.

Responsibilities

• Perform cross continuum care activities across clinics, urgent care, and other sites as appropriate.
• Provide customer service to patient, family, physician/APP, and care team to include discharge milestones.
• Model and maintain professional communications with colleagues and staff.
• Delegate responsibilities appropriately while remaining accountable for outcomes.
• Ensure patients and families experience medically appropriate transitions of care.
