Qualifications

• Master's degree in Social Work from an accredited university.
• Current license in Illinois as a Social Worker, LSW OR proof of a pending license obtained within 90 days of hire.

Responsabilités

• Coordinate person‑centric transitional care plans and manage a complex caseload.
• Provide comprehensive psychosocial assessments.
• Educate patients on levels of health care and entitlements.
• Support discharge planning and continuity of care planning.
• Facilitate communication among physicians, nurses, and care teams.
• Lead and participate in care management initiatives and data analysis.
