Job summary

TAG MedStaffing is seeking an experienced RN Case Manager for an in-office role in Staten Island, NY. The position involves case management, discharge planning, care coordination, and utilization review within an acute care hospital setting.

The ideal candidate has 2+ years of hospital-based RN case management or discharge planning experience, with strong clinical assessment, communication, and interdisciplinary collaboration skills. Active NY RN license and PRI certification are required.

Qualifications

• Graduate of an accredited nursing program is required.
• Active New York State Registered Nurse license is required.
• Active PRI certification is required.
• 2+ years of RN case management experience in hospital/acute care.
• Strong knowledge of care coordination, discharge planning, utilization management, and interdisciplinary patient care.
• Experience with Meditech and CarePort is required.

Responsibilities

• Coordinate patient care across the continuum using clinical guidelines and individualized care plans.
• Assess patient needs, barriers to care, and clinical requirements to support discharge planning.
• Collaborate with physicians, nursing teams, social services, and other professionals to facilitate timely patient care.
• Coordinate post-acute services and transitions based on patient needs and recommendations.
• Support appropriate utilization, reimbursement, and continuity of care through communication with third-party and internal teams.
• Maintain accurate case management documentation in clinical systems.
