PharMerica is seeking a Pharmacist to join our Hospital/retail team in Texas. You will evaluate orders for safety and dosing, advise on therapy changes, maintain resident profiles, and ensure monitoring labs are in place while collaborating with physicians and nurses.

Qualifications

• Bachelor’s Degree in Pharmacy required; Pharm D preferred.
• One year experience in long term care desired.
• Active pharmacist license in state of practice.
• Knowledge of drug therapy guidelines and regulatory standards.

Responsibilities

• Evaluate physician medication orders for appropriateness, dose, interactions and route.
• Provide clinical recommendations to physicians and staff for therapy changes.
• Maintain patient/resident profiles with meds, labs and history.
• Ensure monitoring labs for high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses and other professionals for support.
• Document communications and interventions in patient records.
• Enforce policy for safe handling, dispensing and management of meds.
• Comply with state/federal pharmacy regulations and CMS/DEA/FDA rules.
• Report medication errors and adverse events to Pharmacy Manager.
• Educate clinical team on safe med handling procedures.
