Job Description

The PRN Case Manager provides as-needed care coordination services to support patient throughput, discharge planning, and quality outcomes in a hospital setting. This role collaborates with the interdisciplinary healthcare team to assess patient needs, identify potential barriers to care, and facilitate appropriate discharge plans. The PRN Case Manager supports patient care activities based on variable scheduling, patient volume, and operational demand, while promoting efficient resource utilization and continuity of care. Work is performed under the supervision of departmental leadership and in accordance with established clinical and operational guidelines, while upholding the mission, vision, and values of MSRH. The PRN role requires flexibility to work variable shifts, including on-call coverage and call-back as needed.

Responsibilities
  • Conduct patient assessments to identify clinical, psychosocial, and discharge planning needs.
  • Collaborate with physicians, nurses, and the interdisciplinary team to support coordinated patient care.
  • Participate in care discussions and contribute to discharge planning activities.
  • Identify and address barriers to patient throughput and timely discharge.
  • Assist patients and families with discharge planning, including coordination of post-acute services such as home care, rehabilitation, or community resources.
  • Serve as a liaison between patients, families, and the healthcare team to support communication and care coordination.
  • Advocate for patients' needs and preferences to support safe, appropriate care transitions.
  • Provide education to patients and families regarding care plans, discharge instructions, and available resources.
  • Coordinate with community agencies and support services as needed to facilitate continuity of care.
  • Monitor patient progress and adjust care coordination activities as appropriate.
  • Document assessments, care plans, and coordination activities accurately and in a timely manner in the electronic medical record.
  • Comply with all regulatory, safety, confidentiality, and quality standards.
  • Perform other job-related duties as assigned based on operational needs.
Qualifications

  • Documents patient care accurately and maintains confidentiality in accordance with regulatory standards.
  • Proficiency in Electronic Medical Record systems and standard computer applications.

Education

Bachelor's in Nursing/Master's Social Work

Required Skills

Medical

UT SELECT Medical insurance is offered free for employees and administered by Blue Cross and Blue Shield of Texas. Family members can be added to the plan through payroll deduction. Employees and their dependents can also receive discounted copays and coinsurance when using UT Health Physicians, a network of 800 premier physicians including more than 100 specialists. Employees receive $50,000 of group term life insurance and $50,000 of basic accidental death and dismemberment insurance for free, with options to purchase additional employee and dependent coverage for both at group rates.

Dental

Three dental insurance plan options are available for employees and their families through Delta Dental Insurance Company, two PPOs and one dental HMO plan. Both PPO plans allow employees to choose any licensed dentist.

Time Off
  • Front-loaded Paid Time Off: 128 to 208 hours (16 to 26 days) of Paid Time Off based on years of service, given at the start of each fiscal year. PTO may be prorated in year one based on date of hire.
  • Extended Illness Bank: 8 hours (1 day) accrued per month which can be used for illness or injury after one day of Paid Time Off is taken.
  • Paid Family Leave: Up to 240 hours (6 weeks) to care for a spouse, child, or parent after 6 months of consecutive employment.
  • Holidays: 12 set paid holidays each year.
