Qualifications

• Active Pharmacist license in good standing.
• Minimum 1 year inpatient hospital pharmacy experience.
• Experience with pharmacokinetic dosing and vancomycin management preferred.
• Epic Willow Inpatient system experience preferred.

Responsibilities

• Work 40 hours per week on varied shifts.
• Cover weekends and evening shifts as required.
• Shift hours from 12:30 PM to 9:00 PM; 7:00 AM to 7:00 PM during 3-shift periods.
• Perform warfarin management duties and vancomycin dosing as part of clinical responsibilities.
• Adhere to facility policies and patient care procedures.

Responsibilities for a PHARM

• Work 40 hours per week on varied shifts
• Cover weekends and evening shifts as required
• Work typical shift hours of 12:30-9 PM
• Adapt to variable weekend hours depending on staffing needs
• Maintain an active Pharmacist license in good standing
• Provide a minimum of one year of inpatient hospital pharmacy experience
• Utilize Epic Willow Inpatient system for documentation and pharmacy management (preferred)
• Apply expertise in pharmacokinetic dosing, specifically vancomycin management
• Perform warfarin management duties as part of clinical responsibilities
• Adhere to all facility policies and procedures for pharmacy and patient care delivery
