Qualifications

• Graduate of an accredited school of pharmacy; BS Pharm or PharmD required.
• Current state license to practice pharmacy (Arizona).
• Two years of hospital pharmacy experience preferred.

Responsibilities

• Verify physician medication orders with accuracy and timeliness; clarify and resolve discrepancies.
• Monitor and evaluate medication management processes; identify risk points and drive quality improvement.
• Verify and approve work by Pharmacy Technicians; provide coaching and feedback.
• Order, receive, and store medications to maintain integrity and compliance.
• Review patient-specific drug therapy for indications, dosing, interactions and contraindications; document interventions.
• Perform anticoagulant monitoring; communicate dose/therapy recommendations per protocol.
• Perform other duties as assigned.
