UnityPoint Health – St. Luke's is seeking a Pharmacist to oversee pharmaceutical care across the hospital, evaluating medication orders, supervising dispensing, and ensuring drug safety and security.

You will provide clinical services including renal dosing, anticoagulation monitoring, and medication reconciliation, while supervising unit-dose and IV admixture processes and supporting education for staff and patients.

Competenze

• Graduate of accredited College of Pharmacy required.
• Pharm.D. Residency preferred.
• One year of hospital experience or Residency preferred.
• Current Iowa pharmacist license or eligibility to practice in Iowa required.
• Valid driver's license required for work-related duties.

Overview

UnityPoint Health- St. Luke's

PRN, as needed, 8-hr. shifts

May include days, evenings, nights, weekends and holidays.

Responsible for all aspects of Pharmaceutical care provided at the Hospital: Evaluates medication orders at verification to promote rational cost effective drug therapy, Supervises accurate dispensing of medications to positively impact patient outcome, supervises technical staff to assure safe and accurate completion of Unit Dose and IV admixture services, supervises medication storage areas both within and outside pharmacy to assure drug safety and security, Provides patient education about new drugs and potential adverse reactions to drugs as requested, sssists with medication reconciliation activities, Applies all Pharmacy related laws and required documentation. Documents interventions in appropriate system. Also provides clinical services including but not limited to: renal dosing program, kinetic & dosing service, anticoagulation monitoring and dosing service. Provides these services to neo-natal, pediatric, adolescent, adult, and geriatric customers.

Responsibilities

• Verifies all orders generated on shift, keeps work flow up to date and current, and cleans up old orders as soon as possible.
• Completes verbal hand off communication to next shift, to ensure continuity of quality patient care and follow through in interventions.
• Screens and affirms appropriateness of orders including allergy check, formulary compliance, pregnancy & lactation status, dosing, duplicate orders etc.
• Documents adverse drugs reactions & medication errors in the RL system as they occur throughout the medical center.
• Fields and investigates drug information questions. Uses resources appropriately and colleagues as needed to ensure timely and accurate responses. Documents as necessary.
• Checks and assures high quality of IV Room products every time. Ensures timeliness of delivery of final product, utilizing means of maximum efficiency.
• Enters TPN orders into compounder computer system accurately and timely.

Qualifications

Education:

• Graduate of accredited College of Pharmacy required.
• Pharm.D. Residency preferred.

Experience:

• One year of hospital experience or Residency preferred.

License(s)/Certification(s):

• Current licensure in good standing to practice as a Registered Pharmacist in Iowa or licensure eligible in Iowa required.
• Valid driver's license when driving any vehicle for work-related reasons required.
