Job summary

Montefiore is seeking an experienced pharmacist to perform all duties without additional support, ensuring documentation and adherence to hospital standards. The role involves comprehensive dispensing, sterile/non-sterile compounding, and participation in performance improvement activities.

The candidate must hold a PharmD (or equivalent) with a valid NYS license and at least three years of hospital experience or a residency, plus ongoing CE credits.

Qualifications

• Pharmacist degree from an accredited school.
• PharmD or MS may substitute for some experience.
• Valid NYS Pharmacist license and registration.
• Minimum three years’ hospital pharmacist experience or completion of a residency.
• Continuing education requirements: 15 CE credits annually.

Responsibilities

• Prepare and dispense medications for unit dose distribution, ambulatory services and sterile/non-sterile compounding.
• Ensure 0 medication errors in routine performance improvement activities.
• Review orders for accuracy and therapeutic appropriateness.
• Address age-specific needs across neonates to geriatrics.
• Monitor drug therapy effectiveness and document results.
• Participate in rounds with other health professionals to discuss drug-related issues.
• Provide drug information to physicians, patients and families.
• Supervise pharmacy staff and interns as assigned.
• Contribute to training of pharmacy personnel and residents.
