Job summary

Montefiore is seeking a licensed Pharmacist to perform full pharmacy duties across unit dose distribution, ambulatory services and sterile/non-sterile compounding. The role requires NEONATES/ pediatrics geriatrics considerations and active participation in rounds and education.

Responsibilities include medication order review, drug information provision, storage inspections, and mentorship of pharmacy staff and interns. Three years of related experience and NYS license are required.

Qualifications

• Must be a graduate from an accredited School of Pharmacy with a BS in Pharmacy; PharmD preferred.
• Valid NYS Pharmacist license and registration required.
• At least three years of related experience.
• Maintains professional competence through continuing education programs and review of periodical literature.
• Maintains and provides proof of 15 Continuing Education credits each year.
• Knowledge of clinical information systems, automated hyperalimentation preparation and automated dispensing systems, automated sterile product systems, FAX machines.

Responsibilities

• Performs all the duties of a Pharmacist position without additional support or assistance.
• Documentation of completed Departmental orientation.
• Prepares and dispenses all types of medications using accepted standards of pharmacy practice for unit dose distribution, ambulatory pharmacy services and compounding of sterile and non‑sterile products.
• Reviews medication orders for accuracy, completeness and therapeutic appropriateness.
• Addresses specific needs and characteristics of neonates, pediatrics, adolescents and geriatrics; can interpret data and carry out interventions appropriate to the patient's age-specific needs.
• Compares current medication therapy with new medication orders to prevent inappropriate drug use.
• Monitors the effectiveness of drug therapy for individual patient cases.
• Participates in multidisciplinary patient care rounds with other health care providers to discuss or explain drug-related issues.
• Provides drug information to physicians and other health care providers, patients or their families.
• Performs a detailed inspection of medication storage areas throughout designated unit/area in accordance with hospital and procedure.
• Assists in the orientation and training of other pharmacy personnel in peer or subordinate relationships. Participates in the training of pharmacy interns and residents.
• Supervises and directs the activities of pharmacy graduates and residents.
• Participation in performance improvement and other appropriate departmental committees. Identifies problems, finds and implements solutions.
• Presents Inservice and/or orientation lectures to Pharmacy, Nursing or Medical staff at least once a year.
• Performs admission drug history and drug discharge counseling for patients or family members as necessary.
• Directs other pharmacy personnel, as assigned.
