Travel Pharmacist Job Details
- Weekly Estimated Pay: $2,520-$2,710
- Location: San Francisco, California 94109
- Setting: Hospital pharmacy; inpatient and outpatient services
- Start Date: November 2, 2026
- Contract Duration: 13 weeks, through February 1, 2027
- Schedule: 5 shifts per week, Monday through Friday
- Shift: 8-hour day shifts, approximately 8:00 a.m. to 4:30 p.m.
- Weekend/Holiday Coverage: None
- Call: None
- Patient Population: Adults; approximate daily census of 12

Hospital Pharmacist Job Requirements
- At least 2 years of pharmacist experience, including inpatient and outpatient practice
- Current California pharmacist license in good standing
- Doctor of Pharmacy (PharmD) degree and Registered Pharmacist (RPh) credentials required
- Transplant pharmacy experience required
- Epic electronic health record and charting system experience required
- Experience with acute care medication management and ambulatory or outpatient pharmacy services
- Strong knowledge of clinical dosing, pharmacokinetic monitoring, renal dosing, anticoagulation, and high-alert medications
- Ability to complete required onboarding, compliance documentation, and applicable orientation activities before beginning the assignment
Clinical Pharmacy Responsibilities
- Provide comprehensive clinical pharmacy services for adult inpatient and outpatient populations
- Review medication orders for appropriateness, safety, interactions, contraindications, and therapeutic effectiveness
- Perform clinical dosing, renal dosing, pharmacokinetic dosing, cardiac dosing, and therapeutic medication monitoring
- Support anticoagulation management, including warfarin dosing
- Prepare and verify IV piggybacks, IV push medications, TPN mixtures, compounded medications, and other sterile products
- Apply sterile technique and maintain compliance with medication safety and hospital pharmacy standards
- Manage high-alert medications and patient-controlled analgesia therapies with careful attention to patient safety
- Collaborate with physicians, nurses, and other members of the interdisciplinary care team
- Document clinical interventions and medication-related care accurately in Epic
