A leading healthcare provider in Fort Wayne, IN is seeking a Hospital Pharmacist to join their team. In this role, you will provide clinical patient care across two hospitals, oversee medication management, and mentor pharmacy residents while working 32 hours a week with a rotating schedule.
Our client, located in the Ft Wayne, IN area, has a need for a Hospital Pharmacist to join their team. You will be working in between 2 hospitals.
This position is 32 hours per week, days with every other weekend.
Eligible for Sign On Bonus and Relocation Bonus.
Clinical generalists rotate through clinical and distribution services, however, anticipate being assigned to a clinical pharmacist role >60% of the month. These services include, but are not limited to, multidisciplinary rounding 7 days per week, oversight, monitoring, preparation, dispensing, and proper documentation of patient medication therapy, as well as advising physicians on matters pertaining to drug usage and control within assigned hospital locations. Providing drug information, patient information, and oversight of information and dispensing processes and systems is also critical. Clinical practices include dosing and monitoring anticoagulants, antimicrobials including vancomycin, total parenteral nutrition, patient-controlled analgesia, and others. With over 160 APPE student rotations a year, generalists have ample opportunity to mentor pharmacy IPPE/APPE students, five (5) PGY1 pharmacy residents and one (1) PGY2 ambulatory care pharmacy resident.
Qualifications:
Candidates must possess a pharmacy degree from an ACPE-accredited institution and have completed a PGY1 pharmacy residency or have equivalent hospital practice experience.
Pharmacy licensure or eligibility in Indiana is required.
Candidates must have a high level of clinical practice skills, enthusiasm for teaching, and the interpersonal skills needed to interact effectively with students, health professionals, and patients.
