Job summary

The Community Hospital Liaison at Denton County MHMR works under the Program Manager of Liaison Services to coordinate case management, crisis response, and outreach across local psychiatric hospitals. The role emphasizes discharge planning, aftercare coordination, and documenting all interactions in the electronic health record.

You will build and maintain relationships with contract hospitals, schedule follow-ups within seven days of discharge, and ensure adherence to Trauma Informed Care

Qualifications

• Bachelor's degree in a human services field required.
• QMHP qualification required or eligible.
• Experience with mental illness and crisis intervention.
• Master's degree preferred.
• Experience in community MHMR or hospital settings preferred.
• Active Texas license as LPC, LMFT, or LCSW required.
• Valid driver's license and auto insurance.
• Out-of-state license holders must obtain Texas license within 30 days.
• Successful completion of training within 30 days.

Responsibilities

• Provide moderately complex case management for discharged individuals.
• Act as liaison between the Center and local psychiatric hospitals.
• Document all interactions in the electronic health record.
• Ensure discharge paperwork is obtained from hospitals.
• Maintain a list of individuals in contract hospitals for follow-up.
• Participate in discharge planning to ensure continuity of care.
• Attend meetings with hospital staff and coordinate treatment teams.
• Provide referrals and coordinate follow-up appointments.
