A leading skilled nursing organization in the Cleveland region is seeking a highly professional Hospital Liaison to support strategic referral relationships and strengthen our presence across local hospitals and post-acute networks. Role Overview

The Hospital Liaison serves as the primary point of contact for hospital partners, ensuring efficient communication, timely assessments, and coordinated transitions of care. The position requires a polished, organized individual who can navigate complex hospital environments, maintain strong professional relationships, and support census development through consistent engagement.

Primary Responsibilities
