Job summary

You will provide and coordinate psychosocial and behavioral services for a defined patient group, work with physicians, nurses and case managers, and support safe transitions and discharges from hospital or clinic.

A crisis intervention presence and abuse/neglect assessment are part of the role.

Qualifications

• Master's degree in Social Work (MSW) from an institution accredited by CSWE.
• Valid Licensed Clinical Social Worker (LCSW) licensure in the State of Florida.
• Professional certification in specialty area.

Responsibilities

• Provide and coordinate psychosocial and behavioral services for their patient group.
• Assess, plan, implement and monitor a plan of care in collaboration with physicians, nurses, case managers and other care team members.
• Advocate for patients and provide direct services, resources, and counseling to support safe transitions and discharges from hospital or clinic.
• Respond to crises and assess for potential abuse/neglect of vulnerable patients.
