Qualifications

• Minimum 1200 hours of inpatient pharmacy experience within the last 5 years (e.g., intern pharmacist, pharmacist) or successful completion of an ASHP accredited clinical pharmacy practice residency.
• Doctor of Pharmacy degree required; California license required at hire.
• Knowledge of Federal and State laws and regulations regarding the provision of pharmaceutical services.
• Experience performing aseptic technique and preparation of sterile IV products and chemotherapeutic agents.
• Excellent verbal and written communication skills.

Responsibilities

• Monitors or manages the medication therapy of hospitalized or ambulatory patients as assigned.
• Develops and implements patient-specific plans to optimize medication therapy in collaboration with the health care team.
• Performs sterile and non-sterile compounding.
• Assists in ensuring accuracy and safety of medication orders and distribution.
