You will manage inpatient and outpatient therapy, oversee staff, review medications, monitor labs, and ensure high-quality, evidence-based, cost-effective care across assigned clinical areas.

Qualifications

• Clinical pharmacists must have residency or equivalent direct patient care experience.
• California pharmacist license is required at hire.
• B.S. in Pharmacy or Pharm.D is required.
• BLS certification is required.
• Strong verbal and written communication skills and ability to input prescription labels.

Responsabilités

• Promote rational and cost-effective inpatient and outpatient drug therapy with physicians and other medical personnel.
• Provide direction to residents, interns, technicians, and other staff.
• Conduct patient medication history inquiries upon referral from physicians and other personnel.
• Develop treatment plans and transitions within care settings.
• Identify and resolve medication-related problems including inappropriate drug choices and adverse reactions.
• Initiate, adjust, and monitor drug therapy per protocol.
• Coordinate and interpret laboratory monitoring of drug therapy.
• Provide patient education on disease state, actions, and compliance.
• Support performance improvement and educate providers and staff.
• Perform other duties as required.
