Job summary

Liquid Personnel is seeking an experienced Adult Social Worker to join the Acute Discharge Team in a hospital setting, delivering person-centred, strengths-based interventions to support timely discharges and safe transitions back into the community.

You will undertake Care Act 2014 assessments, Mental Capacity Act considerations, and work within a multi-disciplinary team to manage risk and ensure high-quality case records.

Qualifications

• MA/BA/BSc in Social Work, Diploma in Social Work or CQSW required.
• Registration with Social Work England is essential.
• Minimum 2 years’ post-qualifying experience in Adult Social Care.

Responsibilities

• Manage a caseload of adults requiring hospital discharge support.
• Complete Care Act assessments and discharge planning.
• Apply Mental Capacity Act 2005 in discharge planning and Best Interests decisions.
• Collaborate with MDTs to ensure safe and timely discharges.
• Maintain accurate case records in line with statutory requirements.
