Hospital Coordinator & Case Manager, Readmissions Reduction Program

This position involves overseeing, coordinating, and administering the Readmissions Reduction Program on-site at hospital(s). It emphasizes relationship building with hospital staff and adapting the program to fit specific hospital settings. Responsibilities include coordinating with hospital staff, participating in rounds, intake of potential participants, managing administrative flow, and serving as a case manager for clients with mental health or substance use disorders as needed.

The role is hybrid, with 70% remote work and 30% on-site at Holy Cross Germantown Hospital in Germantown, MD.

Key Responsibilities:

1. Hospital Coordinator: Coordinate documentation, participate in clinical meetings, ensure data accuracy, collaborate with case managers and leadership, manage data entry, complete intakes, work with emergency and crisis teams, facilitate relationships with hospital staff, and promote Mindoula’s services.
2. Case Manager: Provide resourceful, confidential, and effective support to clients, including counseling, crisis intervention, education, risk assessment, care planning, discharge coordination, and documentation. Collaborate with treatment teams and ensure legal and ethical standards are maintained.

Requirements:

• Bachelor’s Degree in psychology, social work, or related field.
• 2-4 years of relevant experience in behavioral health, case management, counseling, or crisis intervention.
• Knowledge of community resources, social determinants of health, mental health/substance use disorders, and benefits programs.
• Proficiency in Google Suite and Microsoft Office.
• Strong communication, organizational, and multitasking skills.
• Ability to work in a fast-paced environment with a sense of urgency.
