Job summary

Augusta Health is seeking a Clinical Pharmacist to provide decentralized direct patient care across multiple hospital settings, including ICU and pediatric units. You will collaborate with the healthcare team to implement medication programs and develop policies, while precepting students and contributing to the clinical ladder.

Qualifications

• Graduate of an ACPE accredited college of pharmacy with a Doctor of Pharmacy degree, or BS Pharm with commensurate experience.
• Post-Graduate Year 1 (PGY-1) residency or equivalent experience preferred
• Licensed by the Virginia Board of Pharmacy
• Board Certification in Pharmacotherapy or other specialty certification
• Experience in a hospital pharmacy
• Direct patient care setting experience with precepting students and leadership preferred

Responsibilities

• Provide direct patient care across multiple hospital settings (ICU, telemetry, adult medicine, OB/GYN, pediatric and neonatal, surgical units).
• Collaborate with medical staff, nurses and pharmacy leadership to plan, implement, and assess medication-related programs.
• Develop order sets and medication management policies; evaluate practice against clinical guidelines.
• Participate on hospital committees and educate Augusta Health employees.
• Precept pharmacy students and administer competency assessments; manage the medication formulary; prescriber order verification.
• Under supervision of Pharmacy Clinical Manager, support departmental operations.
