Job summary

Qualified candidates will have a Social Worker degree with KS licensure, BLS certification, and 3–5 years hospital experience in Case Management or related fields. This is an hourly, non-exempt position with strong teamwork across disciplines.

Qualifications

• Bachelor's degree in Social Work.
• 3–5 years of hospital experience, including Case Management or related field; skilled and/or rehab units preferred.
• Must have proof of maintenance of current licensure in the State of Kansas.
• Certified in Basic Life Support (BLS).
• Specialty certification strongly encouraged within three years.
• Ability to assess, adapt, and respond to changing and crisis environment.
• Ability to facilitate access to community resources.
• Ability to maintain confidentiality with all aspects of patient care.

Responsibilities

• Coordinate continuation of care and services for patients and families.
• Arrange access to community resources and coordinate with insurers.
• Collaborate with internal and external medical staff and organizations.
• Use critical thinking to assess, plan, provide, and evaluate discharge services.
• Maintain professional standards in Case Management and ensure patient care quality.
