• Evaluates psychosocial and medical information to inform social work action plans and care coordination.
• Documents information in the patient's medical record to communicate psychosocial needs for care in the community.
• Participates in interdisciplinary team meetings and family conferences to coordinate care.
• Advocates for patient access to services and supports patient-centered care.
