Job Title: Hospital Case Expeditor- Atlanta
Pay Grade: K
Job Summary:
Under supervision of the Office of Crisis and Transitions Transition Services Manager, this position is responsible for securing alternative residential placements for individuals with intellectual/developmental disabilities currently being treated in a state hospital/institution, residential treatment program, or other higher level of care setting; coordinating case level information between all parties of the service delivery system in order to expedite and facilitate individual transitions.
Essential Functions:
· Reduce the average length of stay for individuals in a state hospital/institution, residential treatment program, or other higher level of care setting from the date transition activity begins until subsequent transition into the community is completed.
· Coordinate and assemble the transition planning team for each individual assigned to caseload in order to identify the supports, services, adaptive equipment, supervision, and opportunities for community integration that will promote a successful transition for each individual.
· Adherence to the Office of Transitions Guide to Community Transitions from State Hospitals, and Community Transitions from State Hospitals Manual throughout the coordination of each community transition.
· Develop and maintain an inventory of resources available in the catchment area.
· Develop and maintain working relationships with internal and external stakeholders.
Travel Required
Require Qualifications:
Bachelor’s degree in a related field with 4 years of recent related experience, two of which are as a supervisor or lead worker, or a Master’s degree in a related field and two years of related experience, one of which was a supervisory or lead worker, or two years at the lower level or position equivalent.
Demonstrates strong written and interpersonal communication skills with proven ability to provide a variety of formats of high quality, clear and concise documentation.
Preferred Qualifications:
· Experience managing the movement of populations from secure/intensive treatment settings to less restrictive settings.
· Knowledge and experience with Georgia’s service system for individuals with behavioral health disabilities.
