Hospital at Home Command Center RN - Remote, 6 Day Shift

Interested in being on the cutting edge of innovation within nursing? Passionate about caring for patients but no longer want to work in a traditional hospital setting? Enjoy working in a fast-paced and challenging environment? If so, come join our team! The Hospital At Home program is a dynamic care model where appropriate patients transfer to a virtual unit to complete their hospitalization. This program functions like a regular inpatient unit but is set up in the patient’s home, providing medical equipment, meals, and diagnostics in the home setting. The Field RN provides hands-on care and collaborates with a team of clinicians including Command Center RNs, MDs, and Field Therapists such as PT and OT. This role is highly dynamic, requiring close coordination with virtual colleagues to deliver clinical care.

Hospital at Home manages patients meeting inpatient criteria in their homes. Data indicates better outcomes, higher patient/family satisfaction, fewer complications, and shorter stays for these patients. Staff report higher job satisfaction as well. The Field RN plays a vital role in delivering optimal care to this patient subset, with acuity levels comparable to a standard medical floor. This model is suitable for patients meeting specific criteria deemed safe for home hospitalization.

Required Qualifications:

• Registered Nurse License from your resident state within our supported footprint, including:
